There is a box outside the surgery where you can leave written requests.
In order to prevent mistakes, our receptionists are unable to take verbal repeat prescription requests.
If the medication you require is not on repeat please make a prescription request here.
Do you have a carer?
If you are unable to go online and order your repeat prescriptions you can give consent for your carer to do this for you. You and your carer will be asked to complete this short form and sign it to confirm you both agree with the arrangement.
When setting up this service we may need to see photo id for you and or your carer. Please be aware this will give your care access to view your medical records.
How do I order online prescriptions for my child?
Parents with children over 11 will also need to complete this form. If your child is 11 or under we will not need a completed form to set this up for you.
