How do we synchronise your prescriptions?
We need to know how many tablets or capsules of each medicine you have. Fill out this form and return it to us, either by emailing cheshireccg.oaklands@nhs.net or by leaving it in the prescription box at the surgery. We can then work out how many tablets or capsules of each you need to bring the quantities into line with each other.
Will this work for all of my medicines?
This will work best for the tablets and capsules that you take regularly. Prescribed medicines that are taken only “as needed” are more difficult to bring in line. However, please include these items in your count sheet and check the expiry dates.
