Self-referral form for Leighton Hospital Maternity unit
Leighton Hospital’s maternity unit has created a new self-referral form for women to complete once they have found out they are pregnant.
Following submission of the form, women will receive a phone call from a midwife to book their first appointment.
If you are over 10 weeks pregnant please do not fill out the form, please call Maternity Outpatients directly on 01270 273127 (Monday to Friday, 8.30 am – 4.30 pm) to book your first appointment.
If you prefer, you can still call the department as an alternative to completing the self-referral form.
Prior to your booking appointment, please access ‘Screening tests for you and your baby’ information via the following link: https://www.gov.uk/government/publications/screening-tests-for-you-and-your-baby.
It is important to make sure that you are taking Folic Acid 400 mcg (or a higher dose if you have been advised to do so) and Vitamin D 10mg supplements. These can both be found in ‘over-the-counter pregnancy multivitamins’. If you are taking any regular prescribed medication, it is important to check with your GP that it is safe to take during pregnancy.
